Background Checks required of all Mass Youth Adult Members

ATTENTION:  New MANDTORY CORI/Kidsafe form to be submitted by ALL Mass Youth Adult members!
You may fill the online form or print the form out and mail it in to the Mass Youth Office.  There is also a Word version that can be filled out and emailed to: CORI@mayouthsoccer.org

As you all know Mass Youth Soccer is moving to a National Background check as of this September.  The Massachusetts check we used to do will be part of the new national check.

Recently the Massachusetts Legislature passed some new rules regarding Massachusetts background checks and has laid on some new requirements which we must comply with  even though we are moving to the national program.

Mass Youth Soccer MUST now collect from every adult involved in youth soccer a new form.  I am attaching a copy of the form.  The reason, as explained to me, for the new form is to help determine if the person we are checking the background on, is in fact the same person who is part of the Mass Youth Soccer program.  I guess the concern was just providing name, address and date of birth left too much room for duplication and potential error.  The legislature wanted to ensure more information was available to ensure accuracy.

The good news in all of this is that there is still no requirement to collect and provide social security numbers.

We will make every effort  to make it as easy as possible for folks to submit the information to us.  The form will be available on our web site and can be submitted by just completing the form and clicking the send button.  The form will be accessible to print off of our web site, complete and then either fax  or mail to us.

I fully appreciate this is but yet another requirement that must be met, however the intent is to protect, to as great an extent as possible, the children.  A very worthy objective.

We ask your assistance in helping us collect this information.  All of the information on the form is REQUIRED.

Since my note of earlier this morning I have had a number of questions.

1. I am very sorry of the late notice on this change.  I just learned of the requirement last week and given the holiday weekend thought it best to wait until this week to send alone.

2. We have no ability to alter the form.  This is what the Criminal System History Board is demanding of us ( all other agencies who do background checks as well)

3. The forms are due with the start of the new soccer year.   Any adult volunteer who will be working with a program needs to have a form on line.  We are mandated to do the background check.  The theory is that the check should be done prior to the adult volunteer working in the program.  Now theory meets reality and this is generally not possible.  So    AS SOON AS POSSIBLE ON THE FORMS.  Please do the very best you can.  I realize the magnitude of the problems.

4. Accuracy of information.  We trust everyone to submit accurate information.

5. How do the clubs know who has and who has not submitted info to us.  We will have to publish monthly reports to you all on this.

6. The form is one page,  I apologize if 2 pages came through.  I am attaching again and this time its only 1 page.


Good news.  The form only need be submitted once, not every year.

I sincerely appreciate everyone's cooperation and somehow we will get this done.

One final thought ( at least for now);   If  a club wants to collect all the forms from their adult volunteers and then ship those to us, that is
certainly fine by me.  It will save our folks a bit of work.

If on the other hand clubs want their adults to fill out and submit to us, that is OK as well.

The on-line submission should be up and running by the end of the day.
